Welcome to the online application for the Active Learning Award. Here are a few things
to keep in mind as you complete the form:

You may enter the application, save part of your data and re-enter later. To do so,
you must remember exactly how you enter your name and your birth date below.

If you wish to submit a paper copy instead of completing the online application, you
are welcome to do so. Please do not do both.

You must complete at least one activity in each of the three categories of learning
(Discovery, Service and Professional) to qualify for the award. The application will
not let you submit the form until you include something in all three categories. A
single activity may not be submitted for more than one category.

If you believe you have an activity that qualifies for one of these areas, but that
is not shown on the form, you may submit a petition. The petition process is included
within this on-line application if you check one of the petition boxes.

Due dates for the applications are as follows: April 1 for May graduation, July 1
for August graduation, and November 1 for December graduation. Late applications
will not be accepted except under extraordinary circumstances (e.g., illness, death
in family, etc.).

You are encouraged to complete the application as completely as possible. However,
in the event that there is not room for multiple entries of a particular activity
(e.g., only space for one set of contact information for one internship and you had
two internships), please provide information on your most recent (or most relevant)
experience for which you can still provide contact information.
